@Living in the Past hadn't thought about what could be done to improve the design. You raise a good point about the shifter length as this would impact the throw distance between the gears.
Maybe a two-piece design would provide a solution for customization. Seems like a machine shop could make the ball/pivot with a lathe and design it with a threaded end; a cane could be ordered to suit the application. There's a couple problems with this sort of design and I doubt it would be enough of a selling point to sway people from working with what is available from your Toyota parts dealer.
