In addition to the above suggestions, the top of the piston rod should be squared off (red arrow). Place a wrench (preferably ratcheting) on the big nut and a smaller wrench on the the piston rod to prevent it from spinning.

I split my nut with a cutoff wheel in my dremel
 
I use a big pipe wrench on the dust shield and do the same on the bottom one if I dont have an impact gun handy.

Same here. I used a pipe wrench to hold it still while I pounded it off with an impact wrench. Didn't care about the shock anymore, obviously. I also did that on the bottom nut, but the impact wrench twisted the end of the shock shaft off instead of loosening it by it's threads.
 
Seriously...impact gun, comes off instantly

Busting that nipple is always good for a truckload of dirty talk.

About to do mine for the first time and I've been thinking about how I'm going to approach it.

The washer above the bushing is included with new shocks, right?
Maybe use the washer as a sacrificial surface to guide in an angle grinder with a cut-off wheel at the base of the nut.

Actually, if your shocks are OEM, save the shock hardware. it doesn't always come correctly -- or at all -- to fit .
 
I've always had great luck using a high powered 1/2" impact gun on these. Soak it with penetrating lube for a night then zip it off with the impact. Never really had to dick with holding the inner rod since the impact hits it so quickly.
 
I definitely used an angle grinder and just cut it off when I did mine.

Heat then PB Blaster then Gearwrench Ratcheting Wrench then hold Top with Vise Grip or have someone hold shock from below with Channelocks then use impact on Top.
It is indeed a stupid design.

**Wait till you go do the back shocks and snap the bolts off! Make sure there are not kids outside.
 
On omes, i used a ratcheting 19mm and i can't remember what the smaller wrench was, but it fit the stub . some have allen heads up top...they're not always better...i've twisted a couple of allens on fox shocks

i'd like to punch the person who created the stud style shock, then smack the guy who decided to put the left upper one right under the brake master
