What would cause this green roller to jump out of the window roller channel? The symptom of this was that the window wouldn’t roll all the way up. Once I got into the door, I was able to put this roller back into the channel and the window work rolled all the way up.

I’m sure this will happen again if I don’t fix something. What is that something that caused this roller to jump out of the channel?

Is the grease supple ( not tacky / dirty ) in all grease zones, also clean the vertical tracks out. If that’s all good the regulator is possibly bent. Usually when I see that in any other cars shizz is all gummed up. Your stuff looks purdy clean but looks in photos on a tiny phone screen can be deciving.

Already mentioned, something is bent, loose, broken, etc as the the green roller wheel should not come out of the track if everything is correct. Whatever the cause (glass/regulator loose/bent) it should become apparent once you get the door panel off, open the vapor barrier, and watch the regulator and glass move while pressing the up-down switch.

Pull the glass up and ensure there is some sticky tape in the channel. The sticky stuff should keep the glass seated in the channel preventing it from bouncing.
I use used double sided sticky tape.

The common cause is the rubber "filler channel" that sits on the bottom of the glass and fits into the guide channel.
After 29 years, that rubber, and all the rest of the rubber bits on your truck, don't owe you anything more.
What happened is the guide channel now slips on the rubber (or what's left of it) and will likely continue to do so.

Filler channel rubber is still available and inexpensive. I replaced all 4 along with all the window guide rubber when I got my 97.
69971-90D00 You can use the same part number for all 4.

I had this happen on mine. The glass can rotate enough in the tracks and the slide can shift to the right enough for it to come off track.

I ended up using toyota fipg to re-adhere the slide to the glass and greased the track the wheel rides in and solved the issue.

I did all that you mentioned above. I don't see anything obvious of why that roller would come out of the channel other than the channel needs to be moved ro the right as mentioned above. Thanks for your response.

Also check these two bolts on the inner door skin.

If these are loose, or shifted, the regulator can twist/tilt inside the door. The window glass will tilt, the glass won't follow the rear edge of the door frame, and the front roller can come out of the guide channel
That's something that hasn't been mentioned yet. I will check that out. Thank you.

As mentioned, the glass can come up out of the "window channel" that attaches it to the regulator or the window channel
can be loose where it attaches to the regulator:
