Just added my info to survey. In short:
No modifications to truck
275/60/R20 KO2s
~5k miles since install
No rubbing
~10% drop in MPG (to be expected)
Barely noticeable increase in road noise, honestly might just be in my head

Does anyone know if these are OEM or simply painted Tundra wheels? Might be able to get the answer from @LFDOffRoad as it's the pictured LX on their website. They appear more grey than black and like the look of them.

they are definitely Tundra wheels. Some models came in a solid black, I believe the first year or 2 of the TRD Pro came with this wheel in solid black. One of the other TRD packages came with it with a two toned machined finish. Lots of people have powder coated and painted these as well, so could be either.

Spare just fits in spare location like a glove. Was waiting for black ET lugs to come in but had to use chrome ones temporarily. Have since changed them out.

Zero rubbing after mud flap removal and plastic trim and bracket bend on front-rear and fenderwell liner pushed forward on front-front.
Stock otherwise?
 
What is your wheel set up? I have a stock 2010. Took it in for a flat repair and found another tire with bad wear. Trying to see what I can get away with right now as I wanted a slightly bigger tire but I’m not interested in doing a lot of mods at any point.

They are Alpha Epqt Echo's - 18x9 with +25 offset.
 
They put it at 38. I agree with that though. What do you mean by LT

LT means Light Truck, look on the sidewalls of your tires and you will see the size, "LT285/65R18"

The RCTIP (Recommended Cold Tire Inflation Pressure) for those tires on your LC200 is 42psi Front/Rear.

So yes, they are currently underinflated.

In case you are interested, and since you own Toyo tires, here's a link to an excellent monograph published by Toyo Tires about tires in general, and how to determine the appropriate tire pressure if you are running a non-standard size:

You will also find good sections in there about how to identify tire types and what they mean.
