How often are you guys changing this out. I know it's a maintenance item but do you do it at 30k or what. I need to change the heater tees also. I guess do them both at the same time. and same intervals?

PCV 100k or sooner depending on condition. It’s super cheap so not much harm doing it early. And the newer the engine the less often it is needed, since a tight engine will have less blow-by gasses to deal with.

Heater T’s.. some trucks will never need them. Mine are perfect at almost 200k. Others had them pop by 150k. So I wouldn’t assign them an interval, just pick a point that it’s good to get in there and inspect them.
 
I changed mine first at 130k. It was overdue. Still functioning but slow and had a lot of junk/obstruction. I think 100k should be first change. Then every 60k or sooner.
 
Positive Crankcase Ventilation is a good thing.
 
Anything to know before attempting to replace? I have 208k on my ‘10 and am certain mine has not been touched!
 
Be gentle with the foam and you may be able to keep it on. I managed to get just one tear but no separation. I can use the tear for access next time.
Other than that is a simple task. You do need a deep socket.

Every 60-70k miles for me. It's so cheap and easy to replace. Why wait until it is actually having problems to replace it?

Is there an updated part number for 2016+? or is it still 12204-38010 still the correct one?

That is the correct and still current one for the 2008-2021 LX570 and 5.7L LC200.
 
I replaced mine at 80k. It was still working but it's so cheap and easy to replace so why not? Someone in another thread said it takes 10 minutes to replace. I think they must have had one hand tied behind their back because 3 minutes is plenty of time! Mine had some gunky buildup on it but still had a nice click when I shook it.
