Why does it need 2 O2 sensors?

As you can see the wide band isn't working right.

Question .... Are you circulating the water in your heat exchanger to a place it can dump heat or is your system static?

As I understand it, it's a good idea for the wide band and the MAP-ECU to share power and be grounded together.

I know in the MegaSquirt world, all sensors ground back to through ECU ... except the O2 sensor, which needs to go to a common ground point point on the block on a dedicated wire. The ECU will ground to this common point as well. I originally was "sharing" a spliced ground wire to the O2 sensor and it caused all kinds of problems. May be different in this application for sure. I do know that what you are seeing there sure smells like a bad ground for the O2.

As far as the dual O2 sensors... I'd be real curious how the exhaust is run and what the logic is. Most V blocks run 2 sensors because the 2 banks of exhaust are far enough apart to get a good individual reading and can adjust fueling to each bank of cylinders/injectors accordingly. On a an inline 6, the exhaust usually comes together so early there's no opportunity for that level of AFR metering.

Either way, I'm super stoke for you man!!

The exchanger is where you're shedding heat - that water flows to the intercooler (if you look on the engine bay pic you can just barely see the big aluminum box intercooler). Different from an air/air setup. So, yes.

Yep, there's the answer to my muddled up question.

Dude, you won't believe this; I just got an out-of-the-blue email from the founder/CEO of MAPECU asking for an update on your rig. He got my contact info from the tuner I was chatting with in Texas and reached out. His quote .... "We haven't met a toyota ECU we couldn't tame". Am working on proper response for him now.
 
Woah... That's cool! Tell him to fly on out and see for himself. I'm sure my tuner would be honored to meet the guy :)
