Need some help with this. I have a 2009 LX 570 and about three years ago I had the dreaded cabin air filter replacement done incorrectly. The typical symptoms followed, which were lack of memory for the HVA system and delayed start . After searching this form, I found that it was probably the door and it come off the hinge so I reached in there and was able to snap it back and everything‘s worked great ever since.
Fast-forward to a month ago, my HVAC system started doing the same thing so last weekend I took apart the glove box to look inside and sure enough the door hinge had come off from the servo arm. I reattached it and it did not change anything, but it did keep memory, but still has a delayed start. A day later I was driving and I realized that the driver side was stuck in cold even though I put the system in the heat, it was blowing cold air on the driver side. The rear air heat works great and so did the passengers side after looking around I found the servo above the gas pedal and proceeded to remove it and in the process it started working just fine then I focus my attention back to the cabin air filter door and I noticed it wasn’t working so I took that servo apart, checked it and put it all back together. It worked after I installed it but soon after it stopped working.

So here’s my question I feel like there’s something going on other than the servo. I believe they are in perfectly working order. It’s just somethings not telling it to operate correctly. I pulled the code in the HVAC system and cleared the codes that are remaining which were 45 and 46. My question is this an HVAC amplifier or something else? Also, if anyone has a factory service manual, can you tell me which volume has the HVAC system in it because it might be worth my time investment to get it and check to confirm what exactly is out. Any help would be great!
 
I think you still have a problem with the cabin filter door mechanism if you’ve had to fix that 3 times. I wonder if there is a crack or even a part missing you can’t see. I suggest going in there with the fix created by @Oakleyguy and repair it permanently. Then disconnect the battery for a few minutes and then do the HVAC servo reset in this thread.

Thanks for the information. I did disconnect the battery but I don’t have techstream. I guess I need to get it.
I’m about 99% sure the filter door is in good working order and that it’s not cracked or broken. I guess I need to figure out downloading and settings up techstream.
 
If the door is ok then the pin should be looked at closely. They don't just disconnect themselves that many times if everything is totally healthy.
 
Note that the cabin filter hood/arm may be broken but may operate in one direction. At least that was my case… it would close to recirc but the arm was cracked so it wouldn’t flip back to fresh air. Make sure when you’re checking that you watch it open and close multiple times when flipping between recirc and fresh.

Likewise; cycle it back and forth at least 5 times.

My next question is the door servo or blend air servo bad? Also could the whole problem be a bad a/c amp?? If any have more test I can run I will. Any help would be greatly appreciated

I suggest, watching the servo arm on the outside of the housing to watch it move; if it move and the flap does not then the flap is indeed cracked.

You can see it from up looking up underneath, or pull the glovebox out to get a better view. Instructions to pull the box are in my video:

 
Pull the HVAC codes, then reset the HVAC codes. See if that fixes the issue. Sometimes even after fixing physical HVAC problems on Toyota/Lexus platforms you still have to do a reset before things start acting normal. It's super easy to do, see video below:

Codes are 42,45,46. I tried to clear it but the codes stay. Also I’m 99.99% the door and arm are not the problem. I have completely disassembled it and looked it over and it’s not broken.

Your codes point to the flapper/servo assembly still, have you removed the cabin filter, left the door open, and watched the flapper to see if it operates correctly and goes up/down properly when its all fully assembled and in place? Or are you just testing it out in the open without it all reassembled?

Yes I’m positive it’s working when it’s installed. It will work 5-6 times then it stops working. I will say that the ac is keeping its memory but it still delays and only blows cold on the drivers side .
Also thank you for the troubleshooting link. I will study it!!
