Can anyone help me identify this transmission and fuel tank setup?

Yes it's a H2 if it was from a US imported FJ40 and not a H41 from an other market. On the right side of the transfer case will be a stamped date and serial code. First will be a number most likely a 4 or 5. Next a letter that will tell what month. Rest are numbers that tell the number built that month.
 
AFAIK, only ConFerr and Aero ever bothered to add the three vent ports to get EPA approval for their tanks. And since Aero ran bolts through the tank into the bed of the truck, this would be a ConFerr.
