RestoRunnerRepro on facebook makes the faceplate. Based in Thailand I think.

Get it fast. He just started posted again a few days ago for replacements. I had messaged him 1.5 years ago and just got a response.

I believe he said $100 + $20 for shipping, processing, etc.
