So can the current installed shim have an additional shim put in the tappet? Or do I just replace the out of spec shim with a new shim with the corrected height from measuring.

You'll want to remove and replace the out of spec shim with a new shim.
 
Reviving this thread with a valve adjustment-related question. My machine shop cleaned, pressure tested, surfaced, and did a valve job on my cylinder head. They also asked for a new lifter bucket to replace my #11 exhaust bucket. The valve tip had worn a pretty deep groove inside of the bucket and the new bucket (with the original shim) brought that valve back into clearance at 0.011.

My question is, when should you replace the lifter bucket to correct an out-of-spec valve versus replacing the shim to correct the clearance? I have a few other valve buckets with some wear on the underside that you can feel with a pick, but the clearances are just within the range. Should I replace those buckets to prevent them from continuing to wear out? The photo shows the wear on the #11 exhaust bucket and the new replacement bucket below it. Is this something even worth worrying about?

That looks like significant enough wear to me that I would replace them, I would also replace them if you notice any other wear they are relatively cheap if you go to overseas sources. Amayama has them for $11

Thanks, that's what I'm thinking too. Only the #11 exhaust was that bad, but there are a few more that I'll replace just because I've ordered a few spares. After 300k miles, none of them are perfect, but Mud OCD is a hell of a thing.
