Its the TC to trans bolt , no locktight no lock washer
i had the same exact thing happen, i was cruisin down the road and was coming to a stop and heard a "pop" when i went to leave the stop sign nothing happened. funny enough the bolt fell and missed all the gears and i just went in and put the bolt back in with loctite and has been good ever since:D
 
Well after a lot of thought and homework on this forum decided to go with a new TC from Orion with 4-1 gears, got it installed over the past week and wow its a big difference from the stock three speed 2-1 to the 4-1 orion, and also got from George, the twin stick set up, and that is going to be sweet also, heres a couple of pictds of the build

i'm glad you posted the pics of the combo. now i know why you're having a hard time shifting the orion from high to low. your geometry for the upper link isn't great. you'be be way better off lowering the mounting location for the picot bolt. realistically you'd want the rod going from the top of the t-case to the shifter as close to horizontal as possible. that is especially true for the orion since the detent for the range selector is soo firm. the 2-4 is much softer and not as much of an issue.
