Easy.. pull it out throw it away throw the hose away put new one on with new hose.. 5 minutes
 
The grommets can turn to stone from heat and they are very difficult to remove. If this happens you have to be very careful you don't break off a piece and it drops through the valve cover.
 
If you are going to do the PCV might as well do the valve cover gasket, spark plug grommets and PCV grommet at the same time. This is one of those jobs that can go from 5min (PCV only) to a half day if you drop broken pieces of grommet inside the valve cover.

Honestly if it is only leaking a little I'd leave it for now until you gather up parts for the valve cover gasket job.

It's easy, as said. Definitely expect to replace the little grommet and the hose. If it does break off into the valve cover, there is a little baffle underneath there that might prevent the pieces from falling into the valve chamber. You can usually pick the bits out with some tweezers or something.
 
I ordered valve cover gssket and i need to k ow if i got the right one. Does the gasket sit flat on head? The gssket i recieved looks lime it sit with a cone to the gasket
Dont want to unpackage gasket.
Does have round plug grommets.

It fits into a groove in the bottom of the valve cover. Hopefully you purchased a Toyota gasket......
