LC 100 lower Radiator Hose coming off

i have got a LC100 2006 model. Facing issue with the lower radiator hose, it keeps coming off. I have changed the hose 6 months backs and there is no visible damage. When i keep the car under park after an hour or so it seems the pressure builds up and the hose comes off. I have changed the thermostat today still the same.

Issue started when i did a full change of coolant
 
With OEM Radiator, hose, clamp, rad cap, thermostat, reservoir cap & hose and system properly serviced, working & topped. This will not happen!
 
Replace your radiator cap and get a worm gear clamp for now.

I put all new OEM hoses on Aaron a couple months back. I don’t remember them being that textured. Also the there are 2 different constant tension clamps used for the 4 connections of the 2 main hoses. Might want to double check the proper one is being used with an OEM hose. Good luck!
 
My concern is if this is something to just do with the hose and clamp or is there any underlying problem that is creating too much pressure in the cooling system

The radiator cap is supposed to open to the expansion tank when there is pressure over a threshold (16psi?). Even so, your clamp likely is worn out.
 
Pls can anyone help me understand the working of the radiator fan; when its supposed to run and what speed to expect. What i know is mine is not an electrical one it seems mine is a mechanical one.

being you are overseas, which engine is that? 1FZ?
we only got the 2UZ in the US
fan clutch is similar to a torque convertor, it free spins until a certain temp than it locks up and cools below a temp than it unlocks and free spins again
you can change the fluid in the clutch, I prefer to just change the assembly out.

I have change changed the clutch assembly it seems the fan was not rotating properly, now the temp is not going up when idle.
Is the transmission also getting cooled by the same radiator ? I had transmission temp warning when i was climbing a hill
 
The auto trans has a cooler built into the bottom of the radiator. if there was a cooling problem with the fan or radiator, it could overheat the transmission as well
 
Changed Radiator, Thermostat, Radiator Fan Clutch. Still having the issue of temp going up when idle
Today did measured the reading gradually went up from 90c to 110c
Not sure what else to check now
