Hi folks, I have a 2006 lx470, 162k on this rig, owned the truck since 110k miles.
After coming home from a long hike new year eve with my wife we noticed a tapping sound under the hood. Upon further inspection and promptly throwing a misfire code I saw cylinder 1 spark plug stripped the threads and blew out and was the source of the tapping. I had the car towed to my in town Indy mechanic who confirmed. After researching mud I see that Time sert or helicoil is the least invasive option. My shop is saying “ Insert will not work, aluminum guy ( he had an aluminum welder??? Look at the issue ) recommended cylinder head replacement, He said drilling into for the insert will put a lot of shavings inside the engine and didn't want to do that. “ Further goes on to say “ the wall is so damaged the insert won't hold and will tap its way out”.
I’m not a mechanic but I do have an out of state father in law who is a mechanic who says this doesn’t make sense. Should I have my rigged towed to another shop for a second opinion? How could the timesert not work? Why not do the timesert and see if it fails before undertaking a new cylinder head replacement which is going to cost big $. My father in law was saying just put positive pressure air into the intake and blow the shavings/chips back out through the insert hole to prevent buildup in the cylinder. I called a local Napa store and they mentioned this is a fairly common repair. As you can tell I have limited wrenching experience. The pic below is what I saw after removing the spark plug. The last time these plugs were replaced was by Toyota dealer when I acquired the truck and paid for a big $ baseline service with tp/wp…etc. Thanks in advance.

Many options out there. You just need to find a shop that is willing to try. Most shops don’t want the liability of it not working then they are on the dole for making it right. Worst case you try an insert, and if it doesn’t take you’re buying a head anyway.
 
Thank you all. I wish I would have viewed that @2001LC thread back in 2018. I found a shop that will try to put in the time sert insert. I am going to order the correct kit for them. I will followup with result.
 
Knock on wood. I've never had a blowout to deal with. Mostly because I'm big on listen to engine and getting the words out to all I can.
Blowouts, always give plenty of advanced warning tick tick tick pop pop pop. Unfortunately, few INDY shops have a clue.

I'm big advocate of pulling head, have machine shop repair. A senior tech at Toyota, had once said to me: "They're (time sert) 50/50 as to not blowing or blowing. But if cylinder already damaged. Time sert in suti, saves time and money.

I have, inspected them after blowout and time sert installed in situ. Compression was compromised and cylinder scored. Compression in that cylinder, did drop further on subsequent test in one I retest ~30K miles later. I can't say if cylinder was damage already, during blow out or after time sert do to metal falling in. But only cylinder that had issues, was where time sert in situ installed. Nor can I say, was best paratic used during in situ install of time sert.

If I did have plug blow out to deal with. My first step would be, to check/scope cylinders:
  1. Scope cylinders. Compare blowout cylinders to others. All good, including history of engine good (Oil and filter changes, etc. all good.) Pull head.
  2. Sope cylinders. Scoring found in blowout cylinders wall. Time sert insuti. Using best paratic, to keep metal out and remove any that drops in.
Time sert are order, for specific use. They come with tool and proper metal sert for use. i.e. aluminum head.

In the end, it owners money. If engine damaged, due to time sert, and later issues arise. Engine can be replaced.

I have used Time sert in wheel hub for rotor bolts. On the bench, using drill press, which worked very well. Wheel hub, sert is inserted from back side. Which means sert act like a nut on back side. Torquing to 54FT-lbf or more, is not and issue.

Last month, I found a cross threaded bolt in oil pump. We had two options:
  • Time sert in-situ
  • R&R oil pump. PITA big job, bigger than pulling a head.
In situ we'd have 3 consideration:
  1. Getting bore drilling at perfect 90 degree angle. Difficult and a must, as this is the Timing belt tensioner pulley bolt.
  2. Possibility, of dropping metal into crankcase. We could use best practice and since on horizontal plan. So gravity not much of issue. Any large pieces that did drop in, would be filtered by oil pick up tube screen.
  3. Time sert failing, in time. Just read about one in situ of same threads in oil pump, in mud. It was just reported, it fail at 4 yr 62K miles. Our 06LX w/162K miles is a VVti engine. We can't have a T-belt failure in VVti.
We replace oil pump.

Thank you guys for continuing to bring this up. I noticed a very faint tick on cold starts recently and swapped all my plugs this afternoon. Several felt fairly loose in there. I had records of plugs replaced about 110k miles ago by the original owner so it was probably about time anyways. Found some slight cracks on some coil packs which will be next up to replace as I have no history of those being done. Im at 265,000 miles.
 
happened to mine 10+ years ago and 150k miles and no issues with timesert.
Do you remember if you used Loctite red 266 on the external threads of the insert during installation? (I know time sert says it’s only needed on a taper type plug install, not flat gasket type plugs that our cruisers call for)
 
F/u I ended up convincing my shop to install time sert for the fix. So far so good. Kit is 4412E-111, m14 x 1.25, comes with 5 inserts. The remaining spark plugs were only finger tight, and I replaced them all with new denso irridium. I will now be checking the plugs at regular intervals to ensure they do not walk out.
