Best place to get them IMHO
NAPA will cross reference part... remove yours when at store and compare to insure the guy got it right before charging on card and then having to credit, etc, etc. Another good source for HD flashers is from an Automotive Electrical Shop where they rebuild starters and alternators.
I can not find a part number for a stock OEM but maybe CDan will see this and post one up. Not worth calling him and making them pay an 800 charge just for a question.

Thanks for reviving this one. I've been thinking about replacing those side markers for a while, two of mine are cracked. Think I'll be doing this sometime this weekend!

I ordered 2 of each (M1's ) not for my truck but for my trailer... they are Saaaaaw-it !!! 
I put them against my '60 but the holes do not mach they are about 1/16-1/8 off from the stock lamps.

For my truck I did order the BA15 30-LED amber for the front clearance lights (it used to be a faded white) it looks pretty cool with the parking light on and they are super bright !!
For the reverse light I ordered the BA15 5-Watt LED bulb in white. they did not fit as the light housing (bulb) touches the lamp (truck) housing by probably 1mm and keeps the pins from engaging. So I unglued (yeah they are just glued together) the bulb housing from the light housing JB weld it it and put it back together but gave it about 2mm of space and put it in the reverse light.. I hot wire it and its SUPER BRIGHT !!! but have not finish installation yet.... oh yeah I sheared the wires on the second bulb
live and learn.
The mod is easy as long as you dont twist. all you have to do is pull apart straight and is best if you use a screwdriver (a tiny screwdriver) and glue it back with the necessary space for the pins to catch the lamp housing on the rig.
Hope the last part makes sense...

The ones supplied by superbrightleds.com (which they have two versions of) are an almost exact fit. They have ones that either snap into the bases (which are separate) or ones that have screw holes through them and look closer to the original factory ones. In either case the hole are off by about an 1/8" in total and you can make them work with minor adjustments. I went for these and they work fine:

I have the same problem, what gives with the plactic/rubber or whatever they were made of turning black dust.
Mine were totally gone, with the wires hanging inside the engine compartment
Road salt in the winter and sun in the summer will kill most anything rubber with enough time.

The problem I had with mine is because they are only glued, and not very well to begin with, they came apart and broke off after a year of off-roading. I highly recommend that when you get them add more epoxy or JB weld or even crazy glue to where the LED housing meets the actual metal bayonet base. I'm sure the company would have done a warranty replacement for me but since I'm in Canada the hassle of shipping it back and waiting for the replacement was too much of a pain.

I ran a bead of black caulking around the black plastic mounting base and that finished off the look and made them water tight to the body. Have not had a single issue to date.

I still have my stock side marker lights w/ led replacement bulbs and they work great. I got them from CDan for a resasonable price (markers not LED's) - fit perfect of course, and are (replacement LED's) brighter than stock. Unless you are set on the aftermarket ones, that's what i'd do again if i had to.
a couple lessons i've learned though - for my dash i would use less bright LED's in blinkers and 4WD. both just about blind me in the dark. Especially the blinker - damn near had a seizure driving home the other night... between the blinkers and 4WD blinding me the entire way. So either leave stock bulb or get a much lower wattage LED for those.
anyway - good luck guys!!
