I suppose that's a good lesson on why you should always put the cotter pins back in. Let someone else learn the hard way and take notes.

BTW--I'm amazed at how many improperly secured cotter pins there are out there. I swear, one day I'll give a seminar on the TWO correct ways to put in a cotter pin.
