It’s also not new. It’s a remanufactured unit.

Toyota does not sell new starters for older gas engines.

its NEWLY Remanufactured , hence NEW OLD STOCK

Just a heads up, New Old Stock means, new, in original packaging, not remanufactured items that can’t be acquired via any means from the original manufacturer. A new part, that has sat on the shelves for an extended period. If you can still buy the starter remanufactured from Toyota...it’s just a remanufactured unit from the manufacturer.

Back to the topic, the difference Is simple. The above unit, originally numbered 28100-60042, is a direct drive 1.0kW starter. Toyota started manufacturing gear reduction starters with the same output, 28100-60070. Simply put, gear reduction uses less power during startup. For those that are bored, Toyota only remans for our trucks these days like @beno stated and the gear reduction version is 28100-60070-84. The direct drive unit is 28100-60090-84 for the remanufactured version.

Either works. Gear reduction has become more of the industry norm.

Wait, so is this physically smaller than the OEM starter? Is better heat rejection the only improvement, or are there other benefits?
 
Generally speaking, direct drives will be physically larger than the gear reduction starter (-60070). Gear reduction starters will generally be smaller.

As @mattressking stated, gear reduction starters are industry standard for petrol engines.
