I used to use AutoMaintenancePro but it crashed on me and I almost lost all my data. The people were helpful with sending me a database utility program to at least retrieve all my entries and put a pen to paper for posterity, so guess what, now I use a pen and paper and have a special binder setup to keep track. HTH I guess if i ever go get another computer based tracking system, I'll rely more on printing out all entries but then that sorta defeats the purpose.
Microsoft Excel here...got the mileage intervals along the top row and the maintenance items along the first column along with suggested mileage intervals, then just check them off when done.
I highly recommend Automotive Wolf for ~$30. You can keep track of each vehicle, customize maintenance intervals, keep track of intervals, part numbers, mpg, costs, lots of different reports, repairs, etc. I tried the trial version and bought it after seeing all the features. You can even scan your receipts and link them to maintenance or repairs.
I bought a little two subject notebook. I keep my mileage log in the front half and my maintenance records in the back half. The divider between the two sections is a folder, so it makes keeping all my parts/work receipts in the same place very easy. The notebook itself is probably 6" x 8", and it fits in the map pocket on the driver's door very nicely.
I just use a cheap little notebook stuck in the glovebox. Keep track of everything that i do or replace on the cruiser and keep track of oil changes in it so i don't have that ugly sticker in the corner windshield.
I use a Note in MS Outlook, which gets synched to my Palm Pilot. This is a log of actual work done. The detail of it depends on when I remember to put it in! Here's and example.

- changed engine oil at 132k, mobile 5w-30, toyota cheap filter
- 12/31 or about 134k, changed diff and transfer oils, front diff was dirty, grease mix, but not too thick, transfer and rear diff were pretty clean
January 05
- 1/1- greased 6 zirks on drive train
- tried to remove dirve train to do complete clean job, couldn't get it off, sprayed two days in a row with PB Blaster
