if you are a month from purchasing it from the dealer, see if they will take the truck back. worth a try in my opinion.

I'd definitely check and probably replace the fluid. Or at least drain a bit of fluid out and test it. Incorrect fluid level (or extremely burnt fluid) wouldn't shock me.
Also, if I was forced to repair, before I swapped the whole damn transmission and torque converter, given this TSB I'd start with a ECU, since it says "in some cases replacement of the A/T may also be required"... meaning in some (or many?) cases it's not.
The ECU is $1100 retail, at least on the 2016+. I didn't search real hard for the 6-speed but I suspect it's similar. Likely the dealer may need to program it though

ECUs can be reflashed. Which is exactly what the TSB states to do. No idea why replacing what is likely the most heavily tested/validated piece of equipment on the vehicle keeps coming up.
Dealers being scumbags is first thought that comes to my mind.
My specialist shop said the body valve assembly is most likely the culprit, mind you he does a lot of cruisers, sequioas and tundras and this is the first time he saw a 2010 with this kind of issue . FWIF our transmissions are engineered to go into million mile territory so a failure this early is highly unlikely especially on yours, I have 224k on mine. The assembly was $895 shipped, taking it to him next week to install will let you know how it goes. Cheers!

Thanks for this info! Mine is parked for the time being while I put pressure on the Lexus dealership I purchased it from to make this right. There is low probability that will go anywhere but I am pursuing that while I get other opinions and asses other options. Please keep me updated on how things go for you since we seem to have the same issue.
I think my next course of action is a fluid baseline as many on this thread have recommended that and the TSB says that too.
Only the ATF will affect the A/T slipping. But if you're going to keep the truck I'd baseline all the fluids though.
had my order canceled three times now, apparently they are out of stock and no eta on when they come back in stock, you can always buy the transmission assembly which would include this part, but that costs just shy of 3k, waiting on my guy to see if he can source it somehow and will decide afterwards
The valve body replacement did the trick, cost all in was $1220, tranny shifts through all gears much smoother now.

Congrats. I feel bad "liking" a $1200 repair, but compared to the $10k the dealer randomly threw out there, this is good news.
I don't know much about automatic transmissions but it would be really cool if someone who does would disassemble the valve body and see if they can determine the actual failure involved. *cough*@bloc*cough*
BTW the valve body is largely made up of various solenoids, which is what most of the original error codes were detecting as failed. It was probably faster for the mechanic to just swap the whole valve body instead of removing and testing each solenoid (and then trying to locate the exact part required), but I'd bet only one or maybe two of them were actually bad.
The body itself had a crack, the solenoids were reused as yota doesn't ship these with new solenoids these days, my guy mentioned that most all other makes include new solenoids with the body valve. The insane thing he told me was that I literally got the last bv in the country for lord knows how long, they are impossible to find at the moment separately and no eta to when they will restock, both yota and lexus use the same part number. You can buy the whole transmission assembly for about 3k which would include this part but even those are difficult to source with a wait time of 4 to 6 weeks if you're lucky. The lexus dealer would have been able to source the transmission in a weeks time but their cost was $4800 for the part. This fix also cured the rough shifting issues I had experienced at times going from neutral into drive, overall the transmission shifts a lot smoother, similar to my old 100 series.

Thanks for the info and updates. Sorry I have been delayed in following up, just waiting on the the results. @landbruiser200 super happy to hear that the valve body fixed it for you and glad you were able to get one. Here is my update... So after taking mine to the Lexus dealership I purchased it from, they were at first able to see the symptoms (not going past 3rd gear) and dash lights with respective DTC codes (same ones at the start of this thread). They confirmed that the vehicle had the most recent transmission calibration and did a visual inspection of the tranny fluid and saw that it was slightly dark but not burnt or gritty. They did a fluid level inspection and found mine to be slightly overfilled and so they adjusted it and the cleared the DTC codes. They had the vehicle and test drove it on both city and highway road for about 80 miles over 4 days, and even put the vehicle in test mode, and the codes/symptoms did not come back and the vehicle is shifting correctly. I plan on driving it for a long distance this weekend and will try 4 lo too. I am not totally convinced I am out of the woods yet, but so far so good and fingers crossed. The TSB does say to check and adjust fluids first (as many on this thread has said too) but man oh man... if this ends up being the fix and my local Toyota dealership said a new tranny, ECU, and torque converter were needed at $10K--but all it needed was a fluid adjustment--what a difference in outcomes!
@707Cruiser - was hoping to hear the outcome…. Can u pls let us know what ended up happening here, was it just a bad fluid level or something more serious? Thnx I’m experiencing a similar set of codes

@joyRidaz - well, it is a very long story but the short of it is that I had to replace the transmission. After the fluid adjustment mentioned in my last post, the symptoms subsided for the entire summer and fall, then in November the Check Engine, VSC, blinking 4-low lights came back on and the shifting issues at highway speeds resurfaced. It went a little over 3,000 miles before these issues came back. Same DTC codes as before. I cleared the codes, and they came back a week later. So, after having it checked out again we concluded the best course of action was to replace the transmission as per the codes and TSB. I did not replace the ECU, just the tranny.

Oh man I’m sorry to hear that, bummer.
Well I guess I’ll chase down the cheapest options first and hope for the best. Start with the fluid, then investigate the valve body, and finally a remanned or used trans as a last option. Yikes I thought I had an induestructable truck up to 350k miles, I’m “only” at 216k so kinda surprised.
Or the other option and likely winner is to just baby it in the cold and keep clearing codes as they pop up since it seems to drive totally fine before and after the annoying codes.
