2019 28,600 miles Wife was driving down i40 and lost forward gears. Coasted to a stop. Reverse works but a noted hard shift into Reverse is present. Fluid level has been checked at temp and is Full. No codes are present.

Anyone experienced this? I figure it has to be a computer or sensor issue. Could the Lidar/Cruise Control Radar cause this?

Narrow it down to the 8-spd but in the past few years I do recall at least one or two other late-model transmission failures if you want to do some searching. Far from common, but not unheard of.

Just spitballing here, have you checked if the brake pedal was partially engaged and thus the engine output was throttled down.

How does one check this? No brake lights are on and once in awhile it will idle forward until you touch the throttle and then it is done moving forward.

Great news on this one, kind of. It has done it again. Same return trip from Fort Smith and coasted to a stop within 11 miles of where it stopped last time. Wrecker hauled it in, wife is PO'd, and the dealer called and said it actually threw a code.

P0722 Output Speed Sensor Circuit No Signal - Permanent

The service tech did say that code renders the vehicle undriveable and requires an unlock code from corporate to reset it. He said the sensor is likely bad and must be replaced. He said if they clear it now it will just come back. Hopefully this leads to a path of it really being fixed.

Thanks for following up and providing some great information. Now we know at least a potential failure mode

The only experience I have with a Permanent code is on my Ram 2500 Cummins. When the DEF injector or pumps craps it goes into a DEF Failure mode with a Permanent code. In the Ram's case it at least lets you drive 5 mph.

Toyota took this one to the shift into any gear you want and you ain't go no where forward zone.

The Service Tech made it sound like this was common to 8 speeds. Made some comment that they see quite a few on the Sequoia and Tundra....er ah not as many on LCs but then again we don't see many of them.

Here is the one thread on that code specifically I could find, though it's not the 8 speed. Certainly not common.

No issues at all. Normal operation up to coasting to a stop. I did find a rock lodged against the transmission cooler that had abraded the cooler fins slightly. I replaced the cooler but this had absolutely nothing to do with this failure. The previous trip to the dealer with the same symptoms they couldn't pull any codes, checked the transmission fluid, dropped the pan and performed a servo check, then installed pan and refilled with fluid and it drove fine for a month.

The dealer called this morning and stated the issue was not the transmission but is a failed computer module within the back of the transfer case. They have ordered the part, will have to pull the transfer case and remove the tail shaft to replace it. This is covered under warranty.

I think it is worth noting if a 8 speed coasts to a stop and has reverse it may be wise to point the dealer at the transfer case and not the transmission.

I have edited the title of this thread.

A computer within the tail housing of the transfer case? That would be quite a departure from the 6-spd setup..

I wonder if they are referring to the 4L/CDL actuator assembly..

Will post copy of paperwork when it's fixed.

p0722 code - step # 1 is to replace the transfer case shift actuator (not a computer).

The dealer has replaced the shift actuator in the transfer case and the p0722 code returned. They consulted with the Toyota engineers and the next step is the replacement of the transmission. Transmission is ordered and will take about 10 days before it arrives.
