So I have a 150 Prado with a 3L 1KD engine and 120k miles.
For the last few months there has been an intermittent rough idle with a P1605 code.
It happens when the engine is warm, in drive and the brake pedal is down (or handbrake). The engine starts to shake and a small hissing noise from around the EGR accompanies the shake. It goes away in park (no load). A/C etc makes no difference.
I have taken it to 2X Toyota dealers. The first said they can’t help if it does not happen when there (it did not). And the second investigated and could not find anything except for a near out of spec injector value. The issue here is they were all replaced at 85K miles which is very much premature.
As it is intermittent it never does it at the dealer. They spent the day working on it and it only occurred when I arrived back home and stopped in the driveway. It is a harsh shudder every couple of seconds and when severe the revs jump slightly. Super intermittent only happens every couple drives.
The dealer said they checked the EGR function and for vacuum leaks and found nothing. Quoted £5k for injectors without a whole lot of confidence in it fixing the issue.

Techstream readings attached. Already replaced a cracked vacuum hose and the cracked map sensor filter. All service items are done. When MAF was replaced or cleaned issue was significantly worse for the first drive.
Really want to avoid spending £5k to replace injectors that are 1/3 way through their expected life with no guarantee of it helping.
