Is this an annual thing, for those with non-exempt trucks, outside of CA or are we just lucky?

Yep, annual here for any truck made after 75. You can spring for the classic car plates so you only need to do it every 5 years, but I'm a masochist and I like red plates to go with my red truck, so it will be every year for me. At least in my county they no longer do a visual, it's just what comes out of the tailpipe.
 
A lot of places have discontinued the "testing". With the price of gas it pays to keep your rig running its best. If you can't afford that good chance you can't afford to drive much either.

I ran the numbers once, a single B52 sorte has a larger carbon foot print than my life.

If they cared about the air they would mandate syncrozied stop lights and priority traffic flow plus flashing red/yellow at off peak hours.
 
Keep it under 5000miles a year and it's a free for all.
Put classic or antique plates on it and it's a free for all.
