Can't be completely sure without a CO2 cranking test (I like to perform this test before condemning a cat)and I'd do a cylinder leak down test if I was doing the diagnosis as misfires can throw off lambda calculator results but if you're not getting a check engine light and it seems to run good I'd be pretty confident that a new cat would get you to pass.
 
Crap, just realized that yours is a 91 so forget what I said about the check engine light(yours wont throw codes for a misfire) but still, if it runs good I wouldn't suspect a misfire.
 
Okay, If it seems to run good I'd shop around at the local muffler shops to see what they'd charge to replace the cat. Its OBD-1 so California is less picky about the cat you can/can't use than they would be if it were OBD-2 but don't be surprised if its a bit pricey. I have seen some direct fit bolt-in CA approved cats but theyre around 3 times the price of a non-CA one.
 
The one I linked to earlier is under $200, CA legal, may require welding. I'm gonna go for smog hot w/o cat on Friday and go from there.
 
Failed retest. Ran Seafoam through tank, replaced plugs, and drove about 45 min at freeway speeds and then right into test bay. Went down 11 ppm of hydrocarbons at idle but still 50 over pass. So something's going on at idle. Is there and ICV on the 3fe, I'm a little more used to my 02 Tacoma.

Oh, and reading the manual tonight I'm seeing that the spark plug gap should be .043, I had read somewhere else here on the forum to set the plug gap at .0315 (I set to .032). Not likely to have been the only reason it failed but I bet it didn't help.
 
Update - I adjusted valves yesterday and while plugs were out I rechecked compression. The truck was fairly cold and I was using a new Bosch compression tester. Three cylinders were at 150, one was at 155 and another shows 160 so all's good.
