See the circled part. It will come off and acts like a nut. It will catch on to the bolt sticking up on the floor. There should be another nut like that.

do you actually pop that circle piece off the pedal or do you place the pedal over the studs and use a screwdriver or coin or something to turn those circle pieces? Sorry for the lame question, I just don't want to break this new $70 pedal.

Do the second method. Make sure you go between the two symmetrically.
 
I only have experience with the "glue it on and hope the buyer doesn't notice" trick.

I just installed the orange boxx metal dead pedal. it's made for a 4 runner or fj cruiser, but it generally fits in the same area. it's made of metal and you can find their lifetime warranty marketing on their site.

the marketing material says it's for a GX470, but this is untrue. the stud pattern for the fj and the 4 runner are shorter than the same stud pattern for the GX. that being said, I installed this all metal dead pedal with 1 fastener and it's enough. far stronger than the factory unit with 2 plastic fasteners. their system is robust and pretty clever.

I can't say that this is a must buy. it's just a possible option if you want a slight compromise.

for reference, only the bottom stud will line up and that's the one I used to mount.

as can be seen in the photo, some corners of carpet holes can be seen. again, it's not a 100% solution, but it is a final solution.

I recently got two new plastic bolts to secure the foot rest back to the floor. The part number is

I just installed one in my 2003. Studs were in the floor and in good shape. I got the entire assembly with both clips. Took the clips off the footrest by pressing the 2 tabs on each with 2 small flat blade screwdrivers and used my third and fourth hands to pull the clip out of the footrest. Once removed I threaded those onto the studs and then clicked the foot rest onto the clips. Top clip went in no problem. The bottom one was a bit tricky as the carpet was keeping the tabs in the clip from seating. Had to stomp on it a couple times to get it to seat. Right as rain now. Way better with than without!

I just pressed the whole pedal onto the studs :banana::D

Might have done something wrong but it seemed too hard to separate the clips (if that needed to be done, why did it come assembled?) and the hole on the clip that the stud goes into has some kind of locking lip on it so I figured it was just to press the whole pedal on. Seems firm, but I'll report back if I ruined my $70 piece of plastic.
