Thinking of getting this off of MAF... anyone have any pics / advice / install instructions? I'm wondering if you need to weld on the top 'plate'...

Rear, you'll need spring perches or some such on top of the axle to hold the plate flat.
? The upper perches in the rear sit right on top of the axle tube, no welding or other modifications required.
Are they worth it? I don't know. I've run them for awhile but I'm not a hard core wheeler. My thinking was less to hang up given that I'm running fairly small tires (31s about to be 32s) and don't plan on going much larger. But I don't know if it's really made a difference for me. If you were sprung under and really wheeled hard it would probably be worth it.
I only installed mine on the front - I'd be wary of balancing a flat plate on a round tube and expecting it to stay put under torque - but to each their own I guess. Personally, I'd rather have a flat spring perch on top of the axle tube to keep the plate in place.
I think there is a difference between the front and rear axle. The front axle has "ears" for the u-bolts, and the 4+ plate sits on these ears. The rear axle does not have these same ears, and the stock u-bolts go over the axle, not over the "ears." However, the pic of the 4+ flip shows that for the rear there is a provision to help the plate sit solidly on top of the axle tube.

I guess you better, my '82 60 has nothing on the top of the axle tube. It uses round u-bolts over the top of the tube.
I was basing my recommendation to the OP based on experience with my rig.
The top plate in the rear flip kit is not flat. It is not a flat piece of stock balancing on the tube. Due to recent knee surgery I can't crawl under the truck right now or I'd snap a pick. Sorry.
