I think low quality fuel and old fuel could ba a cause to clog up injector.
leaky injector could cause misfire and leaked fuel keeps burning
Then the port gets too hot....etc
just a thought, not exactly sure what had happened

Also at least to my knowledge , my model 05 4runner 2uz-fe doesnt have a conventional fuel filter.
there is a screen on the fuel pump it self.
wouldnt hurt to retrofit a fuel filter
 
To me it looks like the one valve not seating caused the combustion/compression like a continuous back fire and melted the manifold.
 
yes, that scenario could be the culprit
I have since put everything back and the vehicle is in running condition, after the bank 2 head job.
test drove it on highway, grocery shopping, different errands etc,
I am though still dealing with 2 fault codes that won't away
P0038 P0058, which are for down stream AF sensors / heated circuit being faultys
swapped with a new sensor, then swapped from bank 1 and bank 2, still getting the same codes
they showed up about 2-3 seconds after engine is fired up.
checked fuses, no problem
I am working on checking continuity from sensor terminals to ECU
I hope ECU is not fried

I have been a BIMMER guy for the past 15 years, I'm sorry to say that Bimmers are much easier to work on
(at lease the era of 2000-2011'ish inline 6's)
