I have a 2013 LX where the power tilt steering wheel decided to stop automatically adjusting every time my wife got in and out of the vehicle.

The steering wheel is currently in its lowest setting, where I hit my knees on it every time I drive.
It will move in or out but not up or down.

Has anyone else encountered this? Is there a way I can take it apart, manually move it higher, then never touch it again?

Not sure but it might be the gears that can go bad. There was a post here a few years back how to source them on ebay for far less than buying the whole thing. It looks like another Lexus is using the same thing. I'm remote so can't find details now.

I will look and see if I can find the thread you are talking about.
All I want to do is move the wheel back to a decent place then disable automatic movement.

The question is: when you activate the switch do you hear the motor turning?

Here is from my notes
Thread: (61) FIXED 2013 LX570 Tilt Steering for $53 --- using LS460 (2007-2009) part from eBay | IH8MUD Forum
He reports this is an exact match to his 2013 LX mechanism.
Prevention
To get many years of longevity, you can turned off "ALL" automated Tilt-Steering functions.
Some settings can be changed on Infotainment "Setting" menu. But found it did not turn everything off, especially if "Easy Exit" feature was turned on.
I used Tech Stream to ultimately turn-off all the automated functions.
Tilt only operates now if...
1) I use the manual Tilt button on Steering column.
2) I press any of the presets for driver settings using the Lexus Memory System
