Weak tie rod end?

I Blew out a relatively new tie rod end making an easy left hand turn on pavement. Did I just get a bad one or could it be a poor installation?

One other notable fact is that the guys that put in the steering rack initially installed the ends on the wrong side, then removed and replaced them on the correct sides.
 
287,000 on first set of tie rod ends, 2 years/37k on the second. I would venture to say that tie rod ends are not re-usable and improper installation should be fix with new parts.

I'm Having tie rod end issues myself. Did you replace with OEM parts or aftermarket?
