I don't know why this happened, but it 100% had not happen since the valve cover job was done. I wonder if fluids spilled on the belt, causing it to squeak etc but here are the part numbers that were used on it:

The job is pretty straight forward and can 100% be DIYed. It just involves removing the fan shroud / fan / removing the belt / remove tensioner.
The tensioner has a bolt on it, that if you use a wrench, and turn it counter clock wise, will expose a small hole where a small Allen wrench can be inserted to hold it in place. Then the belt can be removed (no tension). After that, you can remove the tensioner part itself, which is held in by some bolts. I would definitely use one of those mirrors on a stick to help during this process. I did a post on a water pump replacement which shows / discusses most of this.
