Some of them wear enough to fall apart. So, yeah, they'll be looser if they have been in there for a few years.
 
Yes, if by indexing you mean, marking everything before removing to make sure everything goes back the way it was before and to make sure that the spline yoke flanges and the propeller shaft flanges were 90 degrees out of phase from each other, then the anwser is Yes.
I used a center punch to mark eveything.

I have done many U-joints over the years, and have found that if properly installed very little resistance is felt when moving the flange or yoke from side to side. I have seen them over sqeezed or off center and definete drag or resistance can be felt.
