This is my saga of buying an used lexus smart keycard (creditcard sized) and successfully registering it to my lexus.
I bought a used keycard for my make model year for about 85$ (its cheap because its used - not virgin), make sure you buy the correct keycard which matches the ICC code on your existing keyfobs which are working.
For 2011 LX this is the model I got from Lexus Smart Card Key 89904-50480 / HYQ14ABB (OEM) - https://southeasternkeys.com/products/lexus-smart-card-key-89904-50642-hyq14abb-oem-copy?variant=43866138771663
Once I got the keycard and put a new battery in, it failed registration in techstream as expected as its a used keycard.
I went to Immo Pin Code - Special Offers - Hot Deals! | Immobilizer Pincode Calculator (official website) - Best ICC alternative - https://www.weboctopus.nl and purchased techstream passcode reset tokens and proceeded with the "smart key reset" mechanism in techstream. I have 4 extra token if anyone wants them. DM.
Once I got the 96 char seed code and VIN, I used it on the weboctopus site along with one of the token I had and got the passcode number. I put this passcode number into techstream and it accepted it and asked me to wait 16 mins.
After 16 mins, I had to do a smart key registration (Classic) and select "I am Registering keys and have replaced the ID Code Box and Smart ECU or have performed Smart Code Reset" and then proceed with onscreen instructions to complete the used key registration 1st and all other existing keys. (Make sure all existing keys are all having new batteries and labeled)
Here is the guide/write up I followed for reference:

Smart Keys: Replacement Info & How-To
While doing my research, a post by doctorman on Toyotanation was a big help for me, so I thought I'd try to contribute by making a detailed guide further to their post. You can find much of this information in other threads and throughout the web, however, my experience was very difficult...
