Is there supposed to be some kind on nut on the end of the adjustment lock stud/bolt? Arrow in picture.. I was adjusting this and it was so loose you could just slide the little pivot nut right off the clamp bolt. Not my picture - I borrowed it from another's thread.

To my knowledge no. That long bolt is not a tensioner. Simply a safety catch if the main bolt comes loose. That long bolt in the photo above was used to tighten the belt and as you can see the bracket bent as a result. HTH

Agree with above. At least on my early FJ62 it does not have a nut or anything else on the end of it. After passing through the slot in the adjusting bracket the adjustment lock stud/bolt goes through a threaded hole in the alternator which allows it to be tightened down to lock in the adjustment made by the long adjusting bolt. The part of the bolt that exits the threaded hole in the alternator is smooth not threaded as it passes through the hole in the barrel nut that moves up and down the long adusting bolt. In the photo you posted it looks like someone tried (very hard) to tighten the long adjusting bolt without first loosening the the adjustment lock stud/bolt.
 
no nut at arrow but there should be a nut on the lower pivot bolt, above the engine mount

Upper tensioner bracket is bent.
