Heres a new thread with the latest tech that is close to bolting right in. I had Colorado Car Audio (they do a lot work for Slee) install a Pioneer media unit (MVH-S600BS) and low profile speakers (Pioneer TS-G1620F).

The speakers are low profile, the grills are very conservative, but they were a close fit with no metal cutting. Sounds awesome, everything is pretty simple.

The two guys that do these installs are very experienced and know these trucks.

For references on costs:
Head unit: $110
Four speakers: $100
Installation: $340

It has hands free (works very well),Bluetooth and all that stuff.

Very nice. Can you make the headunit color 1990's Toyota dash green?
 
So, basically they're just top mounted just to the door card? No issue with the new grille hitting the dash in the front? I've poured thru all of the old threads and I'm about to do something similar, but I think I'll be trying to sink some screws into the metal somehow. Bought 4 of the older TS-G1645's off of Amazon for $60 delivered (1 1/2" mounting depth) and a Sony WX-GS920BH head unit (45x4 RMS) and can't wait to get it in, along with removing all of the other old speakers and amps.
