Not common. Only seen three trucks total in the 7 years I have been doing the GX thing.

This happened to my 03 in March of 2016, about a month after I bought it. The part alone is around $2200 new from Toyota. I have my old one still if anyone wants to buy it from me and try rebuilding it.
They have a diagram on how to tear it down, but you will struggle finding the parts to put it back together. It isn't SUPER common, but it is a problem suffered by the GX, 4Runner, 100 series, and a few Tundra's. Bloody sucks when it happens, and bloody expensive! I feel your pain because I went through it almost a year ago. My condolences.

Update - purchased entire new OEM booster and master cylinder (PN 47050-60081) from Bell Lexus of Scottsdale through their eBay store for $2000 including shipping. The guy at Bell Lexus was super helpful and verified the part is covered by Lexus warranty for 12 mo/12k miles. Having it installed for $300. This is a huge savings over the $3000 quoted by the dealer, but still painful! Would rather have spent that money on mods!

Two weeks and $2,253 later, she's back on the road! Complete new master cylinder/break booster and all new fluid.

I have a malfunctioning master cylinder for sale for $100 plus shipping. :)Otherwise it will end up in the dumpster.

I just noticed the reservoir appears to be overfilled. Does anyone know the procedure for checking (i.e. engine running or not, pump pedle, etc)?

Do you have any way of telling what failed (i.e. any visible leaks, etc)? I'd be curious to rebuild one to see if I can, but I don't want to gamble $200 on it since I don't need one. :P

There were no leaks and the pedal was firm. I think whatever went wrong was electronic or the booster motor, but don't know for sure.
