There are a few threads on this but not one that really ties together which keys to get from where, where to have it cut, and how to program it. The FAQ has a thread within a thread that very well covers how to program a master key specifically for a 2004; link is here: 2004 LX470 transponder key programming instructions
The FAQ thread also covers how to program other year models (I guess the '04 is special). The procedure works; be patient, be deliberate, and try again if it doesn't work the first time.
Now, as for keys, I bought this 2-pack from Amazon for $16: https://www.amazon.com/gp/product/B00KTHXKMO/ref=oh_aui_detailpage_o03_s00?ie=UTF8&psc=1
These have chips in them and I had no problem programming them using the steps in the link above. The downside is that these keys don't have remote capability - no remote lock/unlock - but they work fine for old school "stick in the key and turn it" locking and unlocking.
For cutting, I took them to my local Home Depot which has a newer high-speed cutting machine. The guy matched it up to key #35, told the machine "no transponder" when asked, and it cut them in about a minute total. No charge.
So, for $16 bucks and a little bit of my time I got two new master keys. I don't mind not having remote functions, as one is on my spare ring and the other I intend to hide in a tube zip-tied underneath somewhere.
