What do you suspect? possible lean condition burn a hole in the top of the piston?

That's kind of what I'm leaning towards. Perhaps a bad/clogged injector leaned the cylinder out for too long. I'm not a certified mechanic but that is my first line of thought. A foreign object could do the same thing but there would normally be multiple damage areas on the piston if that were the case. It ran really well, never gave an indication of any issues other than sporadic P0402 and P0420 codes.

Of course this happens after I did the PHH and the oil pump seal. Glad I hadn't done the oil pan gaskets yet.

Now the question is why did you lose compression on hole #2?
