What we do is fun, but there are inherent risks when operating vehicles, traveling away from infrastructure, or eating anything I cook. The following ICE (In Case of Emergency) form was created in case a "what if" occurs that is beyond what a bandaid can help with.

I propose that on club events, everyone should have these in the glovebox of their vehicle for themselves and any passenger. If I go down, you will know where to find it. El Prez keeps a list of member contact info, but this is more for real time information if a victim is unable to communicate.

It is pretty simple, I did not want to go overboard with a full doctor's office survey, and there is plenty of room to add anything you think someone may need to know. I also did not specify driver or passenger like similar forms as I see these change periodically and may be a moot point. Open to edits though.

Basically I tell everyone that I have nitro pills in a little silver bottle on my key chain. It goes everywhere. If I'm unable to get to them just put a couple under my tongue and kick me a few times.

Seriously, good to know about the nitro tabs. Kind of why I wanted to put this together, we don't always know what may be a life saving detail. Ideally, we are all best buds and know everything, but that isn't the case in a club setting.

Good idea. May want to put a copy in your first aide kit as well.

A good first aide kit is also a must have AFAIC. We started with an outfitter kit and added a good bit to it. We added stitch kits, scalpel, surgical stapler, some serious pain killers, lidocane<sp> and syringe, Epi pen, rolls of vet wrap. Vet wrap is kinda like a thin Ace bandage that sticks to itself, about 4" wide. It's great for compression and splints etc.

A good first aid kit (even just a boo boo kit that most consider 1st aid) is important too. I also keep some small bills and coins in mine. You may find yourself in need to buy something and a credit card is not an option. The coins are a holdover from when payphones were a thing, but I am still stuck on the idea.

It is easy to go crazy (is one fire extinguisher enough???), at least this is just a piece of paper.
