How many strut top mount kits do I need?

I ordered the Old Man Emu 2" lift kit and metaltech rear coil converstion kit. I understand I probably need new strut top mount (also called top hat) kits. I have seen it for sale as OME part number OMETH002. My question is - how many of these kits do I need? 2 or 4? Another way of asking is do I need these for the front and back or just for the front only? I found it for sale in stock here:

2. One for each front coilover. None needed for rear. Also buy 4runner versions with round hole. GX470 version have D shaped hole for the adjustable shock.
 
This is my own fault but double check that whatever kit you buy includes the bushing that goes in the mount. I didn't pay attention and bought one that was just the mount and nuts.

What does the bushing look like? I bought KYB SM5640 kits and I don't know if it includes the bushing. I am not doing the install myself as I am clearly not experienced in such things.

The bushing should come with the shock. Its part of the shock itself.
 
In the case of the bilstein 6112 only the top bushing comes with the shock. I bought bare strut mounts that did not include anything but the bare metal mount.
