HD-FTE stall low rev's

Hello all.
We have just pulled all of the intake system from the shut off butterfly to the head, it has all be cleaned.
Injectors have been rebuilt by Diesel Care Toowoomba.
Intake heater has been removed.

After clean and rebuild.

When decelerating and coming to a stop (in a manual) the engine rev's drop untill it stalls, but it is not consistant (maybe 1 in every 3 stops).

Now the question
Any ideas what could be causing this?

Also on a 1 minute uphill pull at full throttle it brings on the engine light but records no fault code. It does not go into limp mode. The light goes out after about 20 minutes of normal driving. (It was doing this before the rebuild and clean)
