At 60K miles, we don't usually see spark plugs blow out. Unless:
Not torque in, at minimum until crush washer crushed.
High number of heating and cooling cycles than normal, for the mileage.


Do you have holes in the muffler or before it?
Where you get those spark plugs 60K miles ago?
I ask because even at Advance Auto, which pulls from World Pack. I've seen bad plugs. I suspect, were customers returns. It's one reason I only buy direct for Denso wholesalers. I also buy the TT, as those if bootlegs are easy to spot.

All the spark plugs came out real nice and were in there nice and snug. Unfortunately cylinder four was stuck and this is what came out:

The threads are damaged. I will have to re-thread it. @2001LC mentioned The spark plug tick is real and here is the proof. It should not be ignored.

You're saying stuck in tube, and pulled out coil and plug as one. That, plug was no longer threaded into head?
Also that capture threads in aluminum head. Have been closely examined and they are so damaged, they can no long hold a spark plug?
You heard the tick-tick-tick pop-pop-pop and ignored it, for how long?

Spark plugs walking, is number one reason we see this. But typically with one blowout, at least a few others loose. They'll have a halo around plugs porcelain, between metal & boot. If all 7 others tight, this one was likely left loose when installed. Crush washer can be revealing clue. or Sometimes, will see bootleg spark plugs from China, burn-up and blow out.

As soon as I heard the tick I drove it home. Maybe 2 miles. Yea the plug was no longer threaded in. Came out with the coil as a whole. Funny thing is I never got a code for cylinder #4….only #three and #eight threw the codes
