BJ74 random puffs of black smoke on acceleration

Under normal acceleration my exaust will sometimes puff black smoke over and above the normal amount of smoke. It is always a very short burst, less then a second, and you would never know it was doing it unless you are looking in the rear view. One thing that I should note is that my truck smokes more then your average 13bt. It hasn't always been this bad but it gotten worst gradually over a long period of time. I've run injector cleaner through the system a couple times. Any idea of what's going on?

truck has about 150,000 km on it.
 
During acceleration. The more I put my foot down the better the chance it will puff.
 
Air filer is clean.
 
Might be a dribbling injector then hard to say though without actually seeing and hearing the truck.
 
same thing noticed

I noticed the same small puffs occasionally upon accelerating hard, does not jerk, stumble, feels fine, and pulls hard through power-band. I am at 8300 ft elevation though, and just thought the small puffing was due to that? Clean, new, air cleaner.
 
Would a pin hole in the diaphragm contribute to this at all?
