Ive seen more issues with 200 and 150 transmissions than 100s and 80s. I do think theres a flaw that with some mileage that appears in the valve body. Some cars have occasional reverse but always seem to have solenoid codes. Once cleared they always come back. I do believe if it's caught early the tranny can be saved. Driving it with the codes and flashing 4lo is going to hurt the tranny in my opinion.
I bought the car with no issue and within a couple weeks reverse was occasionally not there. I dont think it was building enough pressure to engage reverse. The codes in the title appeared but I just think those solenoids weren't getting enough pressure to activate.
I felt pretty strongly it wasn't the clutches in the tranny since it was slipping in gear.
My gut was the fix could heave been a cheap reman valve body, a performance built valve body, or new a tranny which Im sure is the answer Toyota would have given me.
I bought this valve body and did mostly because i would be towing and would have extra weight from an overland build.
There were some very good videos made to make the job easy. It wasn't the cheapest but the info and videos made me feel it was worth the extra money. You use your own solenoids.
I put it in and I will say it's best shifting 200 Ive been in and I've been in many. Slightly firm but very precise and clean.
Super happy I did it.

Not every time, but maybe 50% if my rig sits for a couple weeks between starts, it can take a minute to get pressure in reverse.. and when it does come in it’s gradual. Meanwhile forward gears work great. Absolutely no weird behavior other than that, no codes, all normal.

Been like this with no change for the 80k miles I’ve owned it, with a couple full fluid exchanges in that time.

Any thoughts on whether it’s related?
 
I do think you are in the early stages of pressure loss. I have 205k. Have someone run the codes and see if you have some hidden tranny codes. I’ve seen them with nothing on the dash but have. 1 or 2 solenoid codes

Zero codes. I check all modules every oil change.
 
Have you done a fluid level check according to the FSM? This is a low fluid symptom too.

Yes, using tech stream or OBDFusion to monitor trans fluid temp, not the MFI.

As stated it only happens when the truck sits for weeks, and even then not every time. Always temporary.
