I’d try a Remflex gasket. I had the exact same issue, and an OEM gasket would only last a week or two. A Remflex will seal up the irregularities of an older joint.

I hear you on that. Probably the only janky Toyota fastener on the vehicle. I would find something with the same thread pattern with a better head (not Phillips). Stainless if possible. Use a washer if needed. Can be slightly shorter, but not longer.
Going to look at a clean 1990 62 Saturday that looks great in the pics, but current owner doesn’t know any maintenance history and mentioned “light” rust in the fenders.

If the sensor doesn’t work switch to a Remflex gasket. It will take up the irregularities of the pipe for a proper seal. Did the trick for me a couple years ago.
Check for exhaust leaks near the downstream o2 sensor. This fixed it for me when I had the same issue several years ago. Basically, it lets air leak in on the sensor and throws the code. Good luck.
I just knocked the high spots down with a grinder until they cleared. It really wasn’t much. A year and 15-20,000 miles later with no issues. Wouldn’t buy these wheels again because of it, but it worked out.

Ok. Looks like I should check and clean the connection on the actuator. Fingers crossed. It did have oil running out of it as I drained and cleaned the actuator on the bench. I cleaned it again right before installing, but it still could have gotten oily. Hopefully I didn’t smoke anything.
Replaced seal and O-Ring today. All back together and plugged in, but getting a flashing CDL light all the time. Pushing button doesn’t change it and tried unhooking the battery to reset. Any suggestions?
Thanks, Dan! I don’t have wear issues yet, but I noticed I’m getting a little squirrelly over bumps on the interstate. I’m about 8 months post lift, so I’ll be back at the alignment shop now.
I ran those at stock height for a month without issue before raising the rear.
Jstawgn has a good point to check your bump stops. If they are okay, I'd cover the bump stops in chalk and drive it to see if it's impacting the axle.
Lastly, double check your part numbers and the pics above...
Ian is right. I run 30mm (15mm more inboard) offset on a 2" lift with no rubbing. T4R rims should be fine, but I had to clearance my calipers to fit the 17" Tacoma TRD Sport rims.
