04 GX470 with 217k. I’ve been experiencing a “flare” with a brief increase in rpm on an upshift from 3rd-4th only. Really notice it during slower speeds or in traffic. Took it to a friend at Lexus to have a transmission service done, but the issue is still present (not any worse nor better). Transmission feels fine and don’t believe it’s slipping (hopefully not 🥲) as it shifts relatively smooth and doesn’t gear hunt. Haven’t had the chance to get a scanner on it to pull any codes yet but plan on doing it soon. Has anybody had this issue or something similar?

It could be a problem with the valve body, the solenoids, or the actual clutches. The A750F is fairly complex to diagnose, but the process is explained in the FSM linked below (starting on page 966). It's fairly complex and can involve testing solenoids and the pressure in the transmission.

If it is a solenoid or valve body problem, it's actually fairly easy to DIY the replacement of either of those items.
