About a month ago I had the same thing, voltage drop, battery light, electrical glitches. I had to put the car in neutral at every light and rev the engine to keep it from dying. I'm thinking Alternator replacement... But then I remembered an electrical gremlin I had on my FJ55 oh so many years ago. After searching and replacing many things on that Iron Pig, it turned out to be the main grounding wire. I replaced that and never had an issue again. So on my LX470, I went about checking, cleaning and tightening all of the grounds I could find... Paying extra attention to the main one right near the battery. So far the truck has been running beautifully since that free "fix"... Everybody, Check your Grounds!
 
Ok, to finish my electron story - changing my alternator did not seem to change my issue (low voltage at idle and headlights on.) I replaced the positive battery terminal and cleaned, sanded, tightened the negative battery ground (near the battery) and now it is working well. I likely did not need to replace the alternator yet - (178K miles), only needed to fix my cable connections. Such is life.

Thanks for the follow up. At 178k miles you weren't too far off for replacement anyway.
