Mine only has one hole. And incidentally i have 3 nipples, so i dont know whats goin on.
 
Toyota dropped the the second tow hook on the front back in 1975 on the 40 series. Probably because they realized in most cases only one is needed. Besides it's cheaper to only install one.

But why the additional holes? I swear my 60 had drilled holes on the opposite frame horn. And the rear had two sets of holes too, I just put tow hooks back there.
 
Correction, i have holes on both sides on the framehorns, just one hole in the bumper. I think im gonna cut another one, i need 2 hooks, with my lights on the front bumper i couldnt pull from across the truck with only 1 hook, it would break the light off.
 
I'm sure the holes on the other side are for the factory PTO winch which wasn't offered in the US. The winch has extensions just like the FJ55s had an extensions for the factory winch.

I installed a receiver hitch on the back of my 89 in 1993 and I don't remember how many holes were in the back. I do know I had to remove the factory hook to mount it and it seems there were matching holes on the other side of the frame. I don't think Toyota offered a factory hitch so who knows what the extra holes were for.

The other holes would be for the right hand drive version sold overseas. Toyota wasn't planning for aftermarket.

Well I have pics of Non-US 60s that have one tow hook in the back, more like a loop, on the left hand side frame rail. My 60 has those holes plus matching ones on the right hand side frame rail. Universal tow hooks bolted right in, and were far enough forward not to interfere with the bumper. I had a hitch there too, it used one of the tow hook holes on each side and one extra one that was more rearward on the truck.

LOL, so the RHD got left mounted tow hooks, and the LHD got right mounted tow hooks?
 
Correct, most everything is in reverse.
