Another vote for BBB. We bought a high end washing machine a few years ago. It never work from day one. They sent their tech several times and replaced many parts without any success. The machine has some new features, and the tech admitted he is changing parts and hoping one of them will fix it without knowing what the problem exactly. Meanwhile we are hand washing our clothes while trying to unpack in a new house. Lots of fun! The store refused to take it back because their policy is to fix it. I file a complain with BBB. BBB contacted the store, and a few days later they took the washing machine back and gave us a new one.
 
500 is definitely not enough. I wouldn't hesitate to say that you will, for certain, experience form of premature engine failure due to this situation. It might not be noticeable e.g. 1000k miles early, but then again, it could be drastic e.g. 200,000 miles early. Obviously its hard to determine true premature engine failure when you're talking about 1000 miles but you've had your risk of it being much more than that increased drastically due to this situation.

I wouldn't settle for anything less than 100k above current mileage warranty (and that would be my bottom negotiation) or a re-manned engine. I would opt for the mileage warranty if I was planning on selling in the next 5 or so years, and the new engine if I planned on driving till it died. Manufactured engine on a service report will raise red flags if you plan on selling and you will likely have a harder time selling or decreased value.
 
Thanks for all of your advice. I sent an email to the service manager 2 days ago. I have not received a reply so I just sent an email to the owner today explaining what occurred. I am requesting a lifetime warranty for as long as I own the truck.

You may have a little problem getting 3rd party extended warranty coverage, looks like many stop at 150k-175 miles. But leave that to them to figure out. FYI here's a list of the top 10 rated extended warranties: Here

Be firm- put on your Phelps face- stick to your terms and do not flinch. They will tell you no, more than twice or 3x before you get resolution. You will bounce between the GM, owner, back to GM playing good cop bad cop. Do not accept anything less than you believe to be fair and equitable to your terms.
