Silly question, but what is this for and how do I use it. The last truck I had with a choke also had a carburetor and you would pull the choke not twist it. Thanks!

It is a manual throttle knob, not actually a choke (diesels don't use choke). Turn it clockwise at idle and it will turn up the throttle. Commonly used at a cold start to help the engine warm up a little, or during winching to keep the alternator charging up the battery.

They are also great for use on very rough ground when its hard the keep your foot on the accelerator. When using it, keep a light foot on the accelerator pedal to take the strain off the knob as you turn it.
