The wife was complaining the car was running rough
It does sound its usual loud self, but when standing still, and revving to around 1500-1600rpm the whole car and steering wheel started shaking

any ideas where to start looking first?

Motor Mounts could be cracked. Pull off the skid plates and check the condition of the mounts.

Is this all the time? Or when cold start and just off idle up to 1500?

If it’s the latter, it could be a failing coil pack. It won’t give a code until it completely fails.

I noticed earlier, and it was a cold start
After running car for a while on driveway it was no way near as bad, so could be a cold start thing

Idle is a little rough, but the whole car was vibrating around 1500rpm when cold

Just tried again, car has been sitting for 5 hours so is cold

The about 1000-1400rpm it's shakey

I had the symptoms I described 3 times and was rewarded with a failed coil each time. Here’s the catch, it may take weeks of constant driving before it fails. When it finally does, it will run like crap and you will know. The CEL will flash and the truck will really run nasty.

Mine was a slight hesitation, stutter from off idle up to around 1500 rpm when I first started the truck. It seems to go away after the truck gets up to operating temperature, 10-15 minutes in morning traffic. No CEL codes.

You may want to pick up a coil pack and keep it handy just in case. (And a code reader to, if you don’t have one.)

Someone else may have an idea, so don’t take my diagnosis as gospel...

Thanks man.
I got Techstream and may import one so I have it ready!

I only have 140,000 miles... Is this normal
Unless it's been clocked!!

I just saw your truck was a 2005 diesel Amazon. Probably isn't the coil packs since you don't have coil on plugs like gas 4.7 UZJ100s.
