Make sure the air injection system is working correctly. The ABV three way valve is a common problem. Unfortunately its expensive to replace, (find a used one.) There are two check valves that prevent hot exhaust from reaching the ABV. When the check valves fail, the ABV diaphrams get toasted.
 
While I'm on my soapbox, adding s*** to the gas, monkeying with the idle speed and leaving out air filters just masks the problem. I know its a PITA to find the exact cause of an emissions failure, but when its all working, the truck should pass easily - assuming your engine isn't worn out and burning lots of oil. Step by step testing of each component while following the emissions book will tell you which parts are working correctly and DON'T need to be replaced.
 
Can you bypass the control valve as a temporary fix to make sure air injection is functioning during the emissions check?

No way to bypass the control valves. That's sort of the heart of the system. Mine is fried and the air was blowing only into the breather (funny that I had to reroute the entire system to come back and have the same results...) - I blocked the air bypass hose and it forced air into the air inject rail, but if the cat overheats, I'd imagine the whole truck will go up like a roman candle...not good. Plus, it won't reroute the air into the exhaust system when you decel...so you get backfiring.

So, no. No bypass. I've tried it.
 
I went down to the place that checked out my LC when if failed last time and told them the story about failing only NOx then replacing the cat, then failing the other two. The lady at the desk would not look at my car again. She asked if my car was warm when it was last tested. She said to drive for about 10 miles then go straight down and get it tested. After about 15 miles of driving drove straight down, didn't turn the car off and it passed will flying colors. :)

I think it showed that the the lady didn't want to test the car again. She could have taken my money then said nothing was wrong, but instead just told me to drive around first.
